Making Ganga a doctor for the progression in story is perfectly fine.
They could at least tell through some dialogues as to how Ganga managed to do Medicine and specialise in Surgery
Because both the courses are very demanding.
Ganga joined undergraduate Medicine while she was 24-25 that is quite old because ppl who join are 17-18 years old Medicine is too tough a course to handle at that age. I have seen people who were in their twenties joining medicine having a huge problem coping with academics and competing with kids who are so young.
So it is unlikely that she commuted from Jaitser every day as a day scholar that too coping with two small kids one of whom is just an infant when she joined Medicine
Coming to specialisation - Well in medical fields specialisation/PG is not about attending lectures doing practicals PGs are the backbone of the hospital -they do most of the work under the supervision of the faculty.
They have to do 24 hour calls almost 2-3 times a week where they cannot move out of the premises and they have to go for work straight after night shift that means you are working for 36 hours at a stretch.Along with taking care of patienst you have to prepare for seminars case discussions and various other academic sessions.
I know how young doctors who are young parents struggle during their PG time that too when they are staying in the hospital premises.
That's how PG degrees in Medicine are earned.Thye get stipend and they work like dogs for 36 hours at a stretch.
If Ganga is a specialist she would have gone through all this and it is unlikely that she managed just with the help of her husband who himself is busy with his hospital work and MLA duties

The main reason they made Anandi social worker is obviously because they can make her housewife when they want and involve her in family dramas
We all expected her to go for IAS after finishing graduation but instead they brought her husband who is IAS and they didn't know how to deal with the professional life tracks and integrate them into the story so they bumped him off.
No mentione about how funds were generated for SN. How long did it take Anandi to set up the institution? No mention about that either.
