In Stillwater, Minnesota, Wayne McNut wins at a hot dog eating contest and one of the contestants, Slim Jim Morgan, accuses him of cheating. but the crowd boos him down and he walks off. Later, Wayne goes out to his car and gets in. As he eats another hot dog, he hears something outside and looks out, but doesn't see anything. Something then stabs through the back of the driver's seat and Wayne screams in agony... and then deflates.

At the Men of Letters HQ, Sam comes in and finds Dean at the computer. He admits that he hasn't turned up anything on Metatron on the Mark of Cain. However, he has found them a case about the hot dog eater, who went from 300 pounds to 90 pounds. Sam agrees that they should check it out and Dean goes to take a quick bath. Before he goes, Sam asks if it's about what he said about how they're not brothers, and Dean assures him that he doesn't break that easy.

The brothers pose as FBI agents and go to the sheriff's station in Stillwater. Sheriff Donna Hanscum shows them the coroner's report and confirms that Wayne didn't have any enemies. However, he had a rivalry with Slim Jim. However, Donna tells them that Slim Jim had an alibi at the time of Wayne's death. The brothers go to see Slim Jim, who insists that Wayne was a cheat. Dean notices that his girlfriend, Mala, has a bunch of charms on the shelves. Slim Jam explains that she's a gypsy. Sam asks to use the bathroom and Slim Jim warns him that Mala is taking a shower in one of them.

Sam goes upstairs and searches Mala's room, and finds a mojo bag on the dresser. Mala comes out and Sam leaves, taking the bag with him. He goes back downstairs and leaves with Dean. Back at their motel, the Winchesters examine the bag and confirm that it contains strands of Wayne's hair. Sam confirms that it's a putsi bag used by Gypsies to put curses on their enemies. There's a knock at the door and Dean draws his gun and checks. He then opens it and Mala says that they have something of hers. They let her in and she admits that she was having an affair with Wayne. Mala explains that she used the putsi bag as a blessing to help Wayne win. Then they'd use the prize money to elope to Florida and get a quickie divorce.

A woman, Carol, is exercising at a spa, unaware that someone is watching her. She goes to weigh herself and the killer comes up behind her and knocks her out with a dumbbell. As she recovers conscious, something pokes out of her stomach and her weight drops over a hundred pounds in a matter of seconds.

The next day, the Winchesters show up at the crime scene and check the body. The deputy tells them that Donna took a vacation. Once the deputy leaves, Dean points out a strange mark on the corpse's stomach. Sam suggests that it's a suction mark and Dean says that one of them should stay and question the staff. Sam offers to stay, ogling one of the female staff members. However, Dean insists on being honest and tells him that he's weird around women. Dean talks to the staff member, who explains that she gave Carol the key and told her to lock up when she was done. When the woman breaks into tears and reaches for a Kleenex, Dean notices that she has a similar mark on her back.

Back at the motel, Sam tells Dean that there was a mark on the back of Wayne's neck. Dean has confirmed that the trainer lost a lot of weight recently and wouldn't discuss the mark. He's checked and discovered that she went to the Canyon Valley Wellness Spa a month ago. The brothers watch a video by the owners, Larry and Maritza, guaranteeing weight loss with no extreme diet or workout regimen.

The Winchesters head to Canyon Valley and talk to Maritza and Larry. They claim that they're personal trainer and the couple explain that Larry was Maritza's first client. He was overweight and Maritza helped him to lose the weight. Larry says that there's only one trainer position available and asks Dean how he feels about working in another department.

Dean is soon working in the cafeteria. When Sam comes in, they talk and Dean's new co-worker, Alonso, tells him to stop flirting. Sam is off to yoga class while Dean serves tofu.

Donna goes in for a cupping treatment and Maritza explains that sometimes it leaves a mark. She applies the glass cups and the woman dozes off. Once she does, Maritza leans over and extends a long tendril like tongue, attaches it to the woman's back, and starts sucking.

In the kitchen, Dean is texting when Alonso throws a towel at him and tells him to get back to work. Alonso tells him that they eat the same super food that the clients do, but Dean refuses. Undeterred, Alonso brings in some pudding and says that it's the clients' last meal on spa day. Dean starts scooping up the pudding and samples some, and then pockets a cup.

Sam is working a class through yoga and notices one of the men has a suction mark on his back. He checks other clients and realizes that they have the same mark.

Dean is out back eating pudding and finally passes out.

After Sam's class is over, he notices Larry wheeling Donna down the hallway. Dazed, she recognizes Sam and calls him by his assumed name, and he hastily makes an exit. Dean calls his brother sand says that he needs help, and says that he's in the kitchen. Sam finally locates him and Dean explains that he was drugged. He dozes off again and Sam goes to the kitchen and confronts the chef. He insists that the pudding is normal and that he puts supplements in it. Sam takes them back to Dean, who identifies them as roofies. They go to talk to Donna, who says that she's feeling great and doesn't care that she lost 10 pounds in one day. She explains that her husband left her because of her weight and she just wanted to feel pretty again, and Canyon Valley did that for her. Donna wonders why they're there and Dean says that they're undercover investigating the murders. She shows them her new suction mark and explains about the cupping, and confirms that she ate the pudding. The sheriff confirms that Maritza performed the treatment.

Larry talks to Maritz privately and tells his wife that he checked on Sam and Dean and confirmed that they're hunters. He figures that they're to investigate Wayne's death and tells Maritza to get rid of the evidence while he takes care of the problem.

Sam checks the cupping room.

Maritza goes to a refrigerator and starts dumping out tubs of fat. However, she can't resist and starts eating the fat. Dean comes in behind her with his gun drawn and takes her captive. He ties her up and Maritza insists that she's not a killer: she's a Pishtaco, a Peruvian fat sucker. She explains that she's a parasite and would never kill anyone, and that someone else killed Wayne. When Dean wonders who the murderer is, Maritza says that Alonso is her brother.

Larry confronts his brother-in-law and demands to know how he could ruin their operation. Alonso casually admits that he did it and complains that Larry starved him, and Larry orders him out.

As Sam walks down the hall, he hears Larry screaming from the kitchen. He goes in and finds the owner dead on the floor, his throat ripped out. He and Dean break the news to Maritza, who explains that she brought Alonso there from Peru to show him a more civilized way. Alonso almost killed a client during a treatment and they demoted him to kitchen duty. She figures that he's in the basement where he spends most of her time. Sam wonders what they should do with Maritza and she insists that she's on her side. Considering, Sam asks her how they kill Alonso.

Sam and Dean head for the basement, knives drawn. The lights don't work but they find some flashlights and head downstairs. They split up to search and Dean finds the chef, dead. Meanwhile, Alonso attacks Sam, dropping a box on him. Sam manages to throw him off and grabs his flashlight, and then taunts Alonso, saying that Maritza ratted him out. Alonso comes back at him and they fight, and Sam loses his knife. By the time he picks it back up, Alonso kicks him through a door and knocks the knife out of his hand. As he extends his tongue, Dean arrives and cuts the tongue off, killing the Pishtaco.

Donna takes over and secures the spa. Sam approaches Maritza and tells her that there was a psycho killer on the loose. He offers his condolences on the loss of her entire family, and Dean calls him over. He says that they'll kill Maritza once the police leave. Sam objects and Dean points out that Sam wanted to keep things strictly business and they kill monsters. Sam explains that he meant strictly business between them and wonders if he deserved to die when he was possessed by Gadreel. Dean considers and then agrees to send Maritza back to Peru.

Later back at the HQ, Dean is drinking when Sam comes in. He points out that he saved Sam at the spa, and that he saved him before, and insists that he did it because it's the right thing... and he'd do it again. Sam tells him that's the problem and he doesn't see the upside to being alive. Dean says that they're together fighting the good fight, but Sam tells him that Dean saved him for himself. Sam says that he was ready to die, but Dean saved him because he didn't want to be alone. Dean asks Sam if he would have saved him if he was dying, and Sam says that he wouldn't.

I have so many issues with this epi. Where do I even begin. I have no love left for Sam and I dnt think anything he does is gonna change it.

stoned dean is so awesome

liked the story and the banter of the brothers but hope they dont drag it too much
